JOB DESCRIPTION: Coordinated Entry Specialist – Fulton County COC  

Reports To: Fulton County Coordinated Entry Program Manager 

Position Status: Full Time (40 HRS)

POSITION SUMMARY:

Leads the ongoing administration of the Fulton County COC coordinated entry system (CES). She or he will organize CES efforts with partner agencies, facilitate team meetings and case conferencing meetings, and track project outcomes. Additionally, the Coordinated Entry Specialist will provide oversight and status reports to ensure consistent targeting. Candidate will coordinate all services pertaining to intake and referral needs. Ensures accurate HMIS, VI-SPDAT, referral compliance and retention and diversion tracking for all programs. Actively supports all aspects of planning and execution for all CES services. 

PRINCIPAL ACCOUNTABILITIES: 

• Oversee the administrative operations of the Fulton County COC CES 
• Provide training and coaching to CES participating agencies and ensure proper utilization of the standardized assessment tools as agreed upon by the community partners.
• Remain informed on CES database updates and train partner agencies on any changes 
• Engage and train new services agencies as they become CES partners
• Collaborate with CES participating agencies to ensure assessments are administered and entered into the database, administering and entering them as needed
• Verify data gathered is complete, accurate, and current, creating reports as needed
• Work with CES participating agencies to resolve any data quality issues
• Assist with facilitating Fulton County COC CES provider team meetings as well as case conferencing meetings
• Monitor program data, documentation of services, and progress toward benchmarks • 
• Complete reports and documentation as required by project funders
• Complete CES assessments as needed as part of training to other agencies 
• Work in collaboration with partner agencies toward the full implementation of centralized assessment, referral, and placement
• Act as Liaison with local social service agencies
• Be knowledgeable about CES in order to answer questions posed by outside agencies
• All other duties as assigned 

QUALIFICATIONS:

• Master’s degree in social service field preferred ( i.e. social work, counseling, child and family services, human services) 
• Bachelor’s degree in social services or related field accepted with a minimum of two (2) years of case management experience or social services experience will be considered. 
• Valid Georgia Driver’s License 
• Strong organizational skills
• Excellent written and verbal communication skills 
• Proficient in Outlook and Microsoft Word and other software packages 
• Flexibility (Position will require some evenings and weekend)

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
